Care assessment before discharge from hospital

The hospital staff should discuss any difficulties which you may face when you return home. It is important that before you are discharged you agree with the arrangements which have been made for you.

The Joint Interface Team at the Royal Shrewsbury Hospital consists of social workers and health professionals and can undertake an assessment of your care needs before leaving hospital. At other local hospitals you will need to be referred to the appropriate local community based team.

The ward nurses should be able to put you in touch with social care staff if you require home care or residential care services when you leave hospital.
