Will the assessment be confidential?

We will ask you as few questions as possible to make an adequate assessment.

Information may be shared by staff within the department or with Health colleagues where it is necessary to secure services. Information will only be passed on to another agency with your agreement unless there are exceptional circumstances.

Information will be recorded about you manually or on a computer and access to these records is carefully controlled. Under the Data Protection Act you have the right to know what information is kept about you and you can ask to see it.
