In 2009 Shropshire Council was chosen by the Department of Health and Connecting for Health to create a Common Assessment Framework for Adults (CAF).
We are working in South Shropshire with:
- Adults with learning disabilities
- Carers
- Joint Community Learning Disability Team
- Oswestry and Shropshire Citizen Advocacy (OSCA)
- Taking Part (Advocacy Service)
- Shropshire Community Council
- West Mercia Police
- ShropDoc
- West Midlands Ambulance Service
- Mayfair Community Centre
- Social care system suppliers OLM Systems
Julie Edgington, Shropshire CAF Lead said
“This is an opportunity to work with service users and carers to promote and enhance information sharing amongst partner agencies. We aim to improve and deliver better life experiences by reducing the need to give information each time a new professional becomes involved in a person’s care needs".
"We will ensure that through sharing information, with consent of the service user, professionals are better informed to make decisions regarding a person’s care needs based on accurate and up to date information”.
