The Common Assessment Framework (CAF) is a three year project funded by the Department of Health.

We will create a Common Assessment Framework for Adults (CAF) in Shropshire to:

• improve information sharing
• help protect and enable adults
• give people more choice and control
• improve communications so agencies don’t keep asking the same questions

We are working with the following organisations in South Shropshire

• Adults with learning disabilities
• Carers
• Joint Community Learning Disability Team
• Oswestry and Shropshire Citizen Advocacy (OSCA)
• Taking Part (Advocacy Service)
• West Mercia Police
• ShropDoc
• West Midlands Ambulance Service
• Mayfair Community Centre
• Social care system suppliers OLM Systems

Project outline

• building upon existing processes such as self-assessment and client-held records to help people take control of their own care
• create an Emergency Action Plan for service users and carers
• develop a virtual record for Police and Health Partners to access
• develop CAF messaging
• reduce the number of times an individual has to provide the same basic information
• ensure that, where an individual provides information and consent to one agency, it can be available, appropriately, to all the health and social care professionals involved
• bring planning of care forward as a result of having better data earlier in the review process
