This form will go directly to the social care team which is nearest to you or nearest to the person you are enquiring on behalf of.

You can use this form to make enquiries about the assessment process or services for older people, people with a physical or learning disability, substance misusers, people with a mental health problem and people with a sight or hearing impairment

The team will respond to you within 10 days.

This form is not a self-referral for services form.
