Care assessments in Shropshire

A care assessment will usually take the form of a meeting to discuss and consider your personal circumstances. It is an opportunity for you to tell us about your circumstances and discuss your care needs. The aim is to reach an agreement about what help can be provided.
  
We ask questions, so that we can build up a picture of what you need to help you live as independently as possible. We only ask questions that are necessary to help decide what help you need.

You will be able to discuss the types of help which would make it easier for you to live as independently as possible. This help may be provided by a number of different people, all working together to meet your needs.
