Who can be assessed?
Everyone has the right to request an assessment to discuss their needs or the needs of their carers.
You can refer yourself for an assessment, or a member of your family may refer you with your permission, by contacting your local Social Care team. You can also be referred for an assessment by any other agency.
Your GP will be able to advise you on the best method of referral for an assessment.
When you contact your local Social Care team they can find out the nature of your needs. If you meet the eligibility criteria you will be referred for an assessment.
If you are not eligible for services you will, where possible, be offered appropriate advice and information.
Who will assess your needs?
Community Services has the responsibility to co-ordinate an assessment under the Community Care Act. Your Doctor, Nurse, Social Worker or Occupational Therapist may all contribute.
