Common Assessment Framework in Shropshire will be created by:

• building upon existing processes such as self-assessment and client-held records to help people take control of their own care
• developing a citizen portal to allow users and carers to record their own assessments and develop support plans
• creating an Emergency Plan for service users and carers
• developing a virtual record for Police and Health Partners to access
• developing CAF messaging
• reducing the number of times an individual has to provide the same basic information
• ensuring that, where an individual provides information and consent to one agency, it can be available, appropriately, to all the health and social care professionals involved
• bringing planning of care forward as a result of having better data earlier in the review process

Our Objectives are to:

• protect and enable adults
• improve information sharing
• help service users to develop their own services
• give people more choice and control
• improve communications so agencies don’t keep asking the same questions
