Common Assessment Framework in Shropshire will be created by:
- building upon existing processes such as self-assessment and client-held records to help people take control of their own care
- developing a citizen portal to allow users and carers to record their own assessments and develop support plans
- creating an Emergency Plan for service users and carers
- developing a virtual record for Police and Health Partners to access
- developing CAF messaging
- reducing the number of times an individual has to provide the same basic information
- ensuring that, where an individual provides information and consent to one agency, it can be available, appropriately, to all the health and social care professionals involved
- bringing planning of care forward as a result of having better data earlier in the review process
Our Objectives are to:
- protect and enable adults
- improve information sharing
- help service users to develop their own services
- give people more choice and control
- improve communications so agencies don’t keep asking the same questions
