Change of circumstances notification

Have your circumstances changed recently?

If we hold details about you and these details have changed, please let us know and we will update your details in all our systems.

You may have moved and therefore have a new address. You may have got married and therefore your name has changed. Or there may be another reason your details have changed.

You can notify us of your change of circumstances by:

• completing the form online (please see link on this page).
• printing and completing the PDF form attached to this page and posting it to us (see contact button for our address)
• writing to us
