When completing the Early Years request for statutory assessment form, please refer to the Special Educational Needs (SEN) Code of Practice sections 5.62-65 or 6.70-73 and the Shropshire Council Revised Guidance for statutory assessment of Special Educational Needs, which are attached to this page.

The six-month review form should be used for pupils of this age range rather than the annual review form. However, for further guidance on conducting reviews of statements, please follow the link below to guidance on the annual reviews page.
