Education and learning Inclusion Services Statutory Assessment (Special Educational Needs)The attached form should be used to request statutory assessment and should provide evidence that appropriate intervention has taken place at School Action and School Action Plus. It should also confirm that parents/carers have been consulted.
Please refer to the Special Educational Needs (SEN) Code of Practice sections 5.62-65 or 6.70-73 and the Shropshire County Council Revised Guidance for Statutory Assessment of Special Educational Needs, which are attached to this page.
