Health and social care Information sharing and assessment (ISA) Multi-Agency Teams in Shropshire
Over the past two years the five multi-agency teams (MATs) situated around the county have had considerable success in enhancing the delivery of early intervention services. The teams consist of co-located practitioners sited within the team bases, and ‘virtual’ members linked to their local MAT. The common systems and processes provide effective integrated working.
There is a Multi-Agency Team in each district borough of Shropshire. These are based at the following areas:
