If you prefer not to use the online form you can print out a PDF copy from the link below and either post it to Shropshire Libraries, at the address on this page, or take it along to your nearest local library.
There are two versions, one for adults and one for anyone under 16.

Data protection statement
Shropshire Council will use all of the information on the form for delivery and improvement of its library services, and will not pass it on to third parties unless legally obliged to do so. The information will be kept for as long as you remain a member of the library.
