Licensing Act 2003 - Personal licences

If you wish to apply for a personal licence you must apply to the licensing authority in the area in which you live and you must be aged 18 or over.
 
When completing the application form, please read the notes at the beginning and end of the form carefully. Complete all sections of the form. If you think a question does not apply to you, please put N/A in the box.
 
If granted, a personal licence is valid for 10 years.

Application for a new Personal Licence

If you wish to apply for a new personal licence you must complete the relevant application form. This form is supplied in the Personal Licence Pack attached to this page.
 
When you complete this form you will need to provide:

  • The fee of £37.00 - cheques should be made payable to 'Shropshire Council'.
  • 2 photographs of yourself in the specified form (see note 1).
  • Either -
    • A criminal conviction certificate issued under section 112 of the Police Act 1997(a), or
    • A criminal record certificate issued under section 113A of the Police Act 1997 or
    • The results of a subject access search under the Data Protection Act 1998(b) of the Police National Computer by the National Identification Service.
    • A basic criminal record check obtainable from Disclosure Scotland. Tel 0870 6096008 or consult their website (see link on this page). Any certificate must be no more than one calendar month old.
  • A completed Disclosure of Convictions and Declaration form.  A list of relevant offences is available at appendix 1.
  • A copy of your licensing qualification.

Licensing Qualifications

The qualifications that will be accepted are:

  • BIIAB Level 2 National Certificate for Personal Licence Holders
  • GQAL Level 2 Certificate for Personal Licence Holders
  • EDI Level 2 National Certificate for Personal Licence Holders
  • HABC Level 2 Award for Personal Licence Holders
  • NCFE Level 2 National Certificate for Personal Licence Holders

Application to renew a Personal Licence

When your licence is due for renewal, you must apply to the Licensing Authority who granted your original licence.  You have two months in which to renew your licence, starting on a date three months before your licence is due to expire.
 
If you wish to renew your licence, you must complete the form Application to Renew Personal Licence.

When you complete this form you will need to provide:

  • The fee of £37.00 - cheques should be made payable to 'Shropshire Council'.  Payments can be accepted by debit and credit card.
  • 2 photographs of yourself in the specified form (see note 1).
  • Either -
    • A criminal conviction certificate issued under section 112 of the Police Act 1997, or
    • A criminal record certificate issued under section 113A of the Police Act 1997, or
    • The results of a subject access search under the Data Protection Act 1998 of the Police National Computer by the National Identification Service. Any certificate must be no more than one calendar month old.
  • Completed form Disclosure of Convictions and Declaration.  A list of relevant offences is available at appendix 1.
  • Your current personal licence or a statement of the reasons of any failure to provide that licence.

What to do if you've lost your Personal Licence, move address or change your name

Notify the local authority in writing who first issued your Personal Licence. If this was North Shropshire District Council, South Shropshire District Council, Shrewsbury & Atcham Borough Council, Oswestry Borough Council, or Bridgnorth District Council, you will need to contact Shropshire Council licensing.

If you still have your Personal Licence, please return both the paper licence and card with your written notification, the fee of £10.50 (cheques to be made payable to Shropshire Council) and a passport-sized photograph.

If you do not still have your Personal Licence, please give the reason why in your written notification - you will still need to send the fee of £10.50 and a passport-sized photograph.

Please note that it is an offence under section 127 of the Licensing Act 2003 to fail to notify of a change of name or address.

Note 1

Two photographs of the applicant shall be -

  • Taken against a light background so that the applicant's features are distinguishable and contrast against the background
  • 45 millimetres by 35 millimetres
  • With full face uncovered and without sunglasses and, unless the applicant wears a head covering due to his religious beliefs, without a head covering
  • On photographic paper

One of the photos should be endorsed with a statement verifying the likeness of the photograph to the applicant by:

  • The chief executive of the licensing justices for the relevant licensing authority (for those with a current justices' licence only) or
  • A solicitor or notary, or
  • A person of standing in the community (such as a bank or building
  • society official, a police officer, a civil servant or minister of religion); or
  • An individual with a professional qualification.
