We want you to remain living independently at home for as long as possible and offer support to your carers.
Social Care provide a wide range of preventive services before considering the option of a nursing home or care home including:
- home care
- day care
- meals on Wheels
- equipment provision to assist with mobility
- adaptations to your home
Some services are provided directly by Adult Social Care and others are provided by private or voluntary organisations like Age Concern.
How do I access Social Care services?
Services may be offered following a care assessment. You can be referred for an assessment by your GP, nurse, or through a self-referral by yourself, a friend or relative. There is a link to more information on care assessments on this page.
