What is NESrT?
A rehabilitation service for people who reside in the North East Shropshire area.
We hope to prevent you from being admitted to hospital or receiving long term care We want to maximise your independence and facilitate early discharge from hospital.
We provide a therapy and rehabilitation service so that you can remain in your own home whilst receiving the support they need.
How does the service work?
Once you have been referred to NESrT we will carry out an assessment with you to identify your rehabilitation needs.
If you are assessed as being eligible to receive services from us your care and specialist needs will be identified. We will spend time with you agreeing on the goals that you would like to achieve from your rehabilitation plan.
Our team includes:
- a Community Liaison Nurse
- a Mental Health Nurse
- a Speech and Language Therapist
- a Social Worker
- a Dietician
- a Medical Practioner
- a Physiotherapist
- a Pharmacist
- Rehabilitation Technicians
- Age Concern Coordinator
- Occupational Therapists
- Administrative staff
- we also have a Maximising Incomes Officer who works with service users for as long as is necessary
In addition to this there is also the opportunity to attend the Bradbury Day Centre, based at the Whitchurch Hospital.
Who can access the service?
We operate an open referral system. This means that you do not need to be referred to us by your GP or a health professional. You can refer yourself or be referred by a friend or relative to receive services from NESrT.
How long does the service last?
NESrT can offer you support for up to 6 weeks at a time.
What happens if I still require care after 6 weeks?
After 4 weeks a review of your care plan takes place. The purpose of this is to evaluate the achievement of goals, plan further care needs and identify services and ongoing needs beyond the point of discharge.
At week 6 a final review is undertaken to ensure that rehabilitation is complete and any further services identified begin.
