People want to remain independent and avoid long term residential care for as long as possible.

START (Short Term Assessment and Reablement Team) is Shropshire County Council's specialist Home Care service providing a short term assessment and reablement service to people living in the community. The team works very closely with service users to maximise their independence and to assess what, if any, their ongoing care needs may be.

START supports people in their own home for a maximum period of 12 weeks. During this time the service-users needs are assessed and a care plan is designed that will maximise the individuals independence so that they can remain in their own home and avoid admission to long term care.

The care plan is reviewed frequently during this period and at the end of the 12 week period the Social Worker will review the plan and if necessary home care will continue through an independent care provider. If this is the case the Social Worker will purchase the care package from the independent sector. If necessary START will work alongside them during the interim period in order to help achieve a successful transition.
