Services available
The multi disciplinary team provide up to 6 weeks short term support for people who are experiencing difficulty managing day to day and aim to enable you to remain at home.
Following a referral to the service a member of the team will carry out an initial assessment in order to identify your care needs.
The team includes:
- nurses
- occupational therapist
- physiotherapist
- social worker
- mental health nurse
The team can also call upon the expertise of an extended team of other professionals including Consultant Geriatrician, Speech Therapist and Dietician.
Who can refer me to the service?
Referrals are accepted from GP's, Health and Social Care professionals and District nurses.
Do I have to live in North West Shropshire to receive the service?
We offer services to people who live in North West Shropshire. Unfortunately we cannot serve people who live in, or whose GP is based in Wrexham or Powys.
