What is intermediate care?

Intermediate care is a term used to describe services provided for older people that enable early discharge from, or prevent inappropriate admission to, hospital or respite care.

Services available

The areas of service provided include rapid response (which aims to avert a crisis due to illness) and community rehabilitation (which aims to return the individual to a maximum level of independence at home).

Service provision in all areas includes a multidisciplinary team working across the health and social care spectrum.

• physiotherapists
• occupational Therapists
• the countywide Short Term Assessment and Reablement Team (START). START is Shropshire Council's in house specialist Home Care service providing a short term assessment and re-ablement service to people living in the community. The team works very closely with service users to maximise their independence and to assess what, if any, their ongoing care needs may be
• a Rapid Response Team
• meeting rooms for use by community groups and projects that are available for hire with or without presentational aids. Please see attachment on this page for hire charges
• an IT suite with internet access available for use by the community
• a cafe that is open to the public and sells a range of hot and cold snacks and beverages, including Fairtrade products. It also offers a buffet and hospitality services. Please see attached menu (please note that this is subject to change)

Cafe opening times:-

Morning and afternoon tea service between 9.30am and 2pm
Lunch service between 12 noon and 2pm.

The centre is located on Roman Road, Shrewsbury SY3 9JN, Tel: 01743 251500. There are spaces reserved for disabled parking.
