We aim to help older people to maximise their independence so that they can stay in their own home.

When a person is temporarily unable to care for themselves, we will try to avoid unnecessary admissions to hospital or long term care through our Rapid Response and rehabilitation services.

Our immediate and long term care teams care and rehabilitation from nurses, carers and specialists such as Occupational Therapists to enable people to remain in their home whilst receiving care and treatment.
