What do South East Shropshire Intermediate Care services provide?
The team provides short term support for people who are temporarily unable to care for themselves and are at risk of being admitted to hospital, residential or nursing care.
Following referral to the service an assessment will take place to identify your care needs.
The team consists of a District Nurse and Social Care carers. Where necessary the team can also provide services from specialists such as an Occupational Therapist or physiotherapist.
Who can refer me to the service?
A GP, District Nurse, Social Worker or the Accident and Emergency Department.
How long does the service last?
We offer care for up to 5 days.
What if I still require care after this time?
A Social Worker will carry out an assessment of your ongoing needs. If appropriate a care package will be organised.
Do I have to live in South Shropshire?
The South Shropshire Rapid Response Team can provide services to people whose GP is based in Ludlow or Craven Arms.
Will I be asked to pay for the service?
There is no cost for the care provided for the first 5 days. If, after this time you still require care the cost of these services will depend on the care package arranged.
