Intermediate Care in Shropshire has developed over the last few years to meet the needs of patients in their local area. The services provided by Intermediate Care Teams across the county include:
- Community Rehabilitation which aims to provide treatment & support to people who require rehabilitation following injury or illness in order to remain at home. The service is provided over a short period of time (up to 6 weeks) to help individuals regain maximum independence.
- Intermediate Care beds aim to provide an environment for treatment & support for people during their illness or rehabilitation if they are unable to be treated at home. This service is provided over a short period of time (up to 6 weeks) to enable people to regain their independent living skills & return home as soon as possible.
- Rapid Response which aims to provide intensive support to people during an illness or crisis. This service is provided over a short period of time (up to 5 days) to treat the individual in their own home and avoid hospital admission where appropriate.
