The Assessment and Care Management for Adults with Learning Disabilities is entering an exciting period of challenge and opportunity and this is a high profile post managing a busy multi-disciplinary team comprising Social Care, Health staff and North Shropshire Day Opportunities. As a Learning Disability service we have grasped the opportunities of ‘Valuing People’ to work actively with all stakeholders.

You will need to demonstrate the following:-

· Social Work, Occupational Therapy or RNMH / RNLD qualification. · Experience of Care Management and Multi disciplinary working. · 3 years Post Qualifying experience, familiar with a structure of staff supervision and performance review. · Experience of multi-disciplinary working with people with a learning disability. · Knowledge of relevant statute and Health service provision. · Leadership and supervision skills. · Financial and performance management experience.
