The Shropshire Adult Social Care (ASC) Preparing for Adulthood Team (PFA Team) was established in 2018. They are a virtual team supporting young people across Shropshire who have adult care and support needs. They work together with a range of partners in the local authority, across health services and third sector organisations to deliver person centred support to young people and their families which focuses on developing independence and living enjoyable, fulfilling lives.
