The PFA Team is responsible for delivering some of our statutory responsibilities regarding social care and supporting young people.

We work together with a range of partners in the local authority, across health services and third sector organisations to deliver person centred support to young people and their families which focuses on developing independence and living enjoyable, fulfilling lives.   

We will:  

• Work with you and those who know you well to understand your support needs
  
• Find out what's important to you, and how you wish to live your life
• Co-ordinate the support you need, focusing on you being as independent as possible
• Regularly review your support to make sure it is still right for you
